Thank you for your interest in becoming a patient of the Greenbelt Family Health Team. We have ENDED our Waitlist as we are now proudly participating in Health Care Connect. This is a program that has been created by the Ministry of Health and Long Term Care to assist you in linking with doctors who are accepting patients.
If you have previously signed up on our Waitlist, we will be calling you over following few weeks to invite you to come in and sign up at our clinic. Please do not call our reception as our staff will be unable to confirm when you will be invited to sign up

Please note, if you are unable to communicate in English or French, you are responsible in obtaining a professional interpreter for your medical visits. If these services are cost prohibitive for you, we suggest registering with your nearest Community Health Centers which provides interpreter services - Click here for information. Our doctors will not accept patients into their practice if language barriers lead to poor patient care and safety concerns.

To become a patient of the Greenbelt Family Health Team, you will need to join Health Care Connect (HCC). If do not currently have a family physician, you will speak to someone who will ask you a short series of questions and sign you up for the program. You can also complete this process online by visiting www.ontario.ca/healthcareconnect
When speaking with Health Care Connect you may identify that you would like to become a patient with the Greenbelt FHT. HCC will refer you based on HCC waitlist process. Once referred to the Greenbelt FHT one of our administrative clerical staff will call you to book an appointment.
PLEASE NOTE: Greenbelt FHT is dedicated to enrolling patients who currently do not have a family physician. If you already have a family physician and want to change over the Greenbelt FHT for a valid reason (example you have moved and are no longer in proximity to your family physician) you will need to de-roster with your current physician. This can be done by calling Service Ontario at 1 866 532 3161. Within 5-10 days you will receive a letter confirming that you are de-rostered with that doctor. Once you have the letter please call HCC
PLEASE NOTE: We can not see you prior to receiving the referral letter from HCC. If an emergency, we recommend that you proceed to the nearest emergency department for care. Greenbelt FHT is unable to provide you with urgent medical care unless you are a registered patient of our clinic
